HOURS: 30 hours per week (0.75 FTE), Benefits-Eligible
LOCATION: Remote, field-based position in Northeast Wisconsin

JOB SUMMARY:

The Wisconsin Women’s Health Foundation (WWHF) is a mid-size nonprofit with a 20-year history of providing high-quality women’s health education and outreach programming statewide. The WWHF is seeking a professional to join our First Breath team as a Maternal & Child Health Educator. First Breath is a statewide program that helps people make positive changes to their tobacco, alcohol, and other substance use during pregnancy and beyond. The Maternal & Child Health Educator will provide evidence-based tobacco treatment and substance use early intervention services to pregnant and postpartum individuals residing in northeast Wisconsin. This position is mostly virtual, with occasional face-to-face visits occurring in participants’ homes or community locations.

RESPONSIBILITIES:

1. Health Education, Counseling, and Case Management (80%)
• Manage a caseload of pregnant and postpartum individuals residing in northeast Wisconsin.
• Provide intensive, individualized tobacco treatment/smoking cessation counseling and relapse prevention counseling to participants via virtual, home/community visits, phone calls, and text messages
• Provide in-home education to family members with the goal of reducing infant and childhood exposure to Environmental Tobacco Smoke
• Perform Carbon Monoxide (CO) breath tests with program participants and family members
• Perform formal screening, brief intervention, and referrals for substance use
• Conduct screening, referrals, and follow-up for other health and social service needs
• Documentation and online data entry of all services on a weekly basis

2. Community Outreach & Engagement (10%)
• Organize and facilitate annual Participant Advisory Groups and other participant engagement activities as determined by the annual workplan, funders, and/or supervisor
• Develop and maintain relationships with health and social service providers in northeast Wisconsin
• Represent the First Breath program and serve as an advocate for participants at local community outreach events and on coalitions and workgroups


3. Program Communication, Operations, and Development (10%)
• Attend and contribute to bi-weekly WWHF agency wide staff meetings
• Attend and contribute to regular First Breath team meetings, including preparing case briefings, preparing and facilitating presentations, and participating in group discussions
• Engage in regular one-on-one coaching sessions with MCH program coordinator
• Engage in regular quality assurance activities
• Respond in a timely manner to all requests for information from MCH program coordinator, MCH program manager, and other WWHF staff
• Provide feedback and recommendations for program development and quality improvement
• Assist in the development of new counseling tools and educational materials
• Ensure the unique needs of women of northeast Wisconsin are represented in perinatal programming at the Wisconsin Women’s Health Foundation
• Engage in ongoing continuing education opportunities and share learned knowledge with the team and organization

REQUIRED SKILLS:

2. Experiencing working with diverse communities and demonstrated cultural humilty
3. Demonstrated ability to handle protected health information
4. Excellent interpersonal communication skills and ability to treat clients with dignity and respect
5. Excellent organizational skills, attention to detail, and ability to multitask
6. Ability to work independently and demonstrated self-starter
7. Strong technology skills including experience with Microsoft Office, data entry software, and text messaging

MINIMUM QUALIFICATIONS:
1. Bachelor's degree in Nursing, Social Work, Psychology, Health Education, or a related field *
2. 5 years of experience in an area related to patient care, case management, health education, and/or counseling *
3. Valid driver’s license, reliable vehicle, and car insurance required
4. Ability to travel occasionally throughout northeast Wisconsin and to WWHF main office in Madison, Wisconsin (employee will be reimbursed for all work-related travel)
5. A fully secure and confidential workspace (computer, phone, printer, and other supplies are provided)
6. Ability to obtain and maintain Certified Tobacco Treatment Specialist (CTTS) within six months of hire, Substance Use Counselor Certification within twelve months of hire, Wisconsin Mandated Reporter training, and other trainings and certifications as determined by supervisor
